Intramural Sports Roster Form

Go to the Adobe Web site to download Acrobat Reader.


Please make sure you:

  1. Fill out roster form COMPLETELY
    1. Names
    2. Social Security Numbers
    3. Class (Freshman, Senior, etc)
    4. Circle best days for your team to play
  2. Make sure writing is legible
  3. Return roster form prior to "due date"
  4. Make sure someone attends captains meeting
  5. Be sure to check eligibility of your players


Eligible Participants:

  1. Any Adelphi student CURRENTLY registered for at least 3 credits
  2. Any Adelphi Faculty or full time Staff Member
  3. In-Season varsity athletes are NOT ELIGIBLE
  4. Intramural office has final ruling on eligibility
