Every student athlete is required to have a medical clearance from Adelphi’s Health Services Center (HSC) prior to participation in a sport.  Please contact the HSC Office to schedule two appointments, one for an EKG (electrocardiogram) and one for an assessment by the Nurse Practitioner.  The EKG will take approximately 15 minutes, and the assessment approximately 30 minutes.  Once completed, you will be given a clearance form to return to your coach.

Please be aware that you must have a history and physical on file from your physician prior to the assessment.  If you have not had this done by your physician, please do so.  If you are unsure, please contact the HSC office at (516) 877-6002 to find out.
